Antiretroviral treatment strategies, complications of antiretroviral therapy, and the use of antiretroviral therapy to understand HIV pathogenesis

Summary:
Antiretroviral therapy has dramatically changed the course of the HIV epidemic, prolonging life and reducing morbidity. However, antiretroviral resistance is common and complications are frequent, requiring alternative treatment strategies and a greater understanding of the etiology and risk factors associated with complications of antiretroviral therapy in order to maintain long-term immunological and clinical benefits.

To meet these challenges, the following studies are underway:

A multicenter trial comparing restoration of immune function and resetting of the viral load set point following antiretroviral treatment interruption, vaccination with a multivalent canarypox vaccine, or both. The impact of these interventions on the latently infected cell reservoir and genital tract compartment is being analyzed.

A study of the association between body fat redistribution and insulin resistance and dyslipidemia in patients on protease inhibitor-inclusive and -sparing antiviral combinations, and the effect of treatment on adipocyte gene expression.

A study of lipoprotein kinetics in patients on antiretroviral therapy

An analysis of the risks for dyslipidemia associated with polymorphisms in host genetics

Studies of new antiretroviral agents, including novel entry inhibitors and protease inhibitors.
