Co-occurance of HIV/AIDS among persons with severe mental illness.

Summary:
This project represents an integration of separate prograrns of research at Penn: mental health services research in the Center for Mental Health Policy and Services Research and AIDS research in the School of Medicine and School of Nursing . The Center for AIDS Research at Penn is one of 21 NIH-funded CFARs and includes HIV and AIDS investigators at University of Pennsylvania , the Children's Hospital of Philadelphia (CHOP), and the Wistar Institute. Among the goals of the CFAR is to create an infrastructure to encourage HIV/AIDS research at Penn and to encourage development of new and emerging areas of AIDS research. This research initiative will help to illuminate the scope of the problem of HIV and AIDS infection among the severely mentally ill, a group at particular risk for infection and for whom treatment access and adherence issues are complex. A longer term goal of this collaboration is to develop an RO1 for an effectiveness trial of a community-based intervention regarding access, quality, and outcomes of care for persons with severe mental illness and comorbid HIV/AIDS and to further develop the capacity for this type of mental health services research within the CFAR. Specifically the project will examine ten years of Medicaid claims within Philadelphia and estimate the prevalence of HIV/AIDS among persons diagnosed with a serious mental illness (DSM IIIR 295.00 - 296.99). Differential prevalence of comorbidity of HIV/AIDS will be calculated for Schizophrenia, Schizoaffective Disorder, Bi-Polar Disorder, Major Depression, Autism, and Obsessive-Compulsive Disorder. Also, the relative risk and attributable risk of HIV/AIDS given SMI morbidity, as well as SMI given HIV/AIDS morbidity will be calculated. Finally, an examination of costs of treatment for each of these groups will complete the pilot study.
