This form should be used to request logos if:

The logo you need is not available for download on the Logos website (access restricted to Health System employees).
You need a logo in a different format (BMP, GIF, TIFF, etc.) than is available on the Logos website (access restricted to Health System employees).
You are not located at a Penn Medicine site, and therefore cannot access the Logos website (access restricted to Health System employees) on the Penn Medicine intranet.

Please allow two business days for your request to be completed. Because of this necessary processing time, please be certain that your logo is not available for immediate download on the Logos website (access restricted to Health System employees)before using this form.

Logos will be provided as email attachments. We will contact you if we have any questions or need any additional information.
