Logo Request Form

This form should be used to request logos if:

  • The logo you need is not available for download on the Logos website.
  • You need a logo in a different format (BMP, GIF, TIFF, etc.) than is available on the Logos website.
  • You are not located at a UPHS site, and therefore cannot access the Logos website on the UPHS intranet.

Please allow two business days for your request to be completed. Because of this necessary processing time, please be certain that your logo is not available for immediate download on the Logos website before using this form.

Logos will be provided as email attachments. We will contact you if we have any questions or need any additional information.

I understand that UPHS logos should be used exactly as provided by the Marketing Department, and should not be skewed, stretched, stylized, rearranged, or otherwise modified. In addition, logos should never be presented in any way that would be damaging to the identity and reputation of UPHS.
