Comprehensive Geriatric Evaluation/Assessment

The evaluation includes a thorough examination, which also considers any existing treatment and medication and an assessment of skills required for independent living.

This thorough evaluation takes place in one or more office visits at either the Ralston Penn Center on the University of Pennsylvania campus, or at our Radnor location, Penn Medicine At Radnor.

Referral may subsequently be made for specialty services such as mental health evaluation, memory evaluation, or social services consultation.

This service is recommended for patients who:

  • Have symptoms that are difficult to diagnose
  • Experience a decline in health or physical function
  • Want to receive all of their care in a single location
  • Are considering a change in living arrangements, such as a nursing home, assisted living residence or moving in with relatives.

Following the evaluation, patients may return to their family physician for care. Results of the evaluation will be communicated with the family physician.

Patients who need primary care may remain with the Ralston or Radnor office to receive all of their care from our geriatric specialists.

Insurance:

The evaluation is covered under most plans.

Managed care patients will need a referral from their primary care physician for the evaluation visits.
