We are deeply committed to providing personal, thoughtful care to each of our patients. Upon your arrival to one of our outpatient offices, you will be asked to register with our receptionists and fill out medical information forms.

Please arrive at least 15 minutes prior to your scheduled appointment to allow time to complete all necessary materials. If you have access to a printer, you can print the forms now and bring the completed forms with you to your visit.

You should also bring copies of recent medical records, laboratory studies, X-rays, CT scans, and MRI’s that pertain to your medical problem. It is also suggested that you bring a list of all current medications taken on a regular basis, including over-the-counter and herbal medications. In addition, it is necessary that you obtain the appropriate referrals from your primary care physician if your insurance plan requires you to do so.

When it is time for your appointment, you will be escorted to an examination room. A member of our healthcare team will ask you about your medical history, followed by a thorough examination by a University of Pennsylvania Orthopaedic Surgeon.

During your visit your Doctor may determine that additional diagnostic tests are required. If x-rays are necessary, they may obtained in the office on the day of the visit. Some studies may require scheduling outside of the office. We will make every effort to help schedule further studies at the earliest possible time.
