Implant Surgery
A one-year full-time academic fellowship is available in Implant Surgery for board eligible Orthopaedic Surgeons. This program is designed to give intensive experience in all phases of Implant Surgery in particular the hip and knee. The fellowship experience is integrated
with the residency program and after appropriate experience and background the Fellow will supervise the hip and knee clinic, as well as the Implant Surgery on the clinic service. There is significant clinical exposure and the Fellow will obtain excellent operating experience. In addition, the Fellow is encouraged to participate in the extensive teaching conferences and research seminars, which are an integral part of the residency program. There is an active laboratory available for Fellows who are interested in pursuing laboratory research. It is anticipated that each Fellow will become involved in research, either on a clinical or laboratory project related to Implant Surgery.

Overview and Objectives
The primary goal of this ACGME accredited Adult Reconstruction Fellowship is to mentor an individual in all aspects of Adult reconstruction. This fellowship in an exciting state of the art experience that has four specialists who work together to provide you with a learning experience of both traditional and cutting edge innovations in the field. Each staff person has a diverse training experience and brings a unique perspective to total joint replacement. All are heavily involved in research as well as clinical pursuits and education. Four of the five workdays are spent in the operating room. The fifth day is a protected day to pursue research interests and gain office experience. Substantial Research opportunities are available with ample support to assist in the production of publishable quality projects. The faculty strongly support the full year rotation schedule, rather than a rotation format, to assure that the experience and relationships with each attending are maximized.
Dr. Jonathan Garino is the current fellowship director. Trained at the New York Orthopedic Hospital and fellowship trained at Penn, he brings a vast array of interests and experience to the program. The fellow currently operates two full days per week with him for the entire year. Dr. Garino is involved in multiple clinical trials and his patients benefit from some of the latest advancements in adult reconstruction. Some of his interests include alternate bearing surfaces such as ceramic on ceramic, minimally invasive exposures, trabecular metal applications, and impaction grafting. Dr. Garino has developed a reputation for complex primary and revision reconstruction which continues to further enhance the overall experience.
Dr. Paul Lotke, as a former fellowship director, continues to play an instrumental role in resident and fellow education both in the operating room and conference. Dr. Lotke’s is recognized as an international opinion leader in Knee reconstruction and has contributed greatly to the orthopedic literature on many levels. His expertise and experience are an invaluable part of this fellowship. The fellow operates with Dr. Lotke one full day a week for the year. Trained at the Hospital for Special Surgery and, along with Marvin Steinberg, was a co-founder of this fellowship nearly twenty years ago. Dr. Lotke’s caseload includes primary and revision knee arthroplasty, hip arthroplasty, unicondylar knee arthroplasty, and patellar femoral arthroplasty.
Dr. Charles Nelson brings a third perspective to the fellowship. Initially trained at Penn, Charles completed the prestigious Adult Reconstructive fellowship at the Hospital for Special Surgery. His expertise is also in primary and revision adult reconstructive surgery, but he has developed special interests in the adult reconstruction in hemophiliac arthropathy, minimally invasive approaches to unicompartmental knee arthroplasty and TA, trabecular metal applications (particularly for patellar resurfacing and complex acetabular revision surgery), alternative bearing surfaces, pelvic, femoral and acetabular osteotomies and hip arthroscopy. The fellow operates with Dr. Nelson one day per week for the year.
Dr. Craig Israelite is the newest member contributing to the fellowship. Following his training at Hahnemann University, Craig became a member of the full time faculty at Hahnemann. While at Hahnemann, he developed special interests in knee reconstruction and he served there as the Director of the Joint Service. In addition to his clinical interests, Craig has always demonstrated a strong commitment to resident and fellow education. He represents a strong addition to our program with a growing role.
Conferences
The fellow is responsible for the adult reconstruction conference that is held on a weekly basis. Case presentations and organized weekly topics make for an educational and enjoyable time. The conference is well attended by the faculty.

Research
This is an ACGME approved fellowship. It has certain requirements that include pursuing a research topic and producing a paper of publishable quality. As mentioned above, each of the faculty is very research oriented. There are many projects that are currently underway and offer an opportunity for fellow participation. The faculty is also supportive of discussing research ideas that may be of interest to the fellow.
Call
The fellow currently does one call for the year, the night prior and day of the orthopedic in training exam.
Duration
The fellowship is 12 months. It extends from August 1st until July 31st. One fellow is accepted each year.
Other Operative Opportunities
All aspects of orthopedics are well represented and the fellow is invited to take advantage of other services of interest when possible. This includes sports, trauma, oncology, hand, spine, foot and ankle, and pediatrics.
Conclusion
The University of Pennsylvania Adult Reconstruction Fellowship is a unique opportunity for someone to increase the breadth and depth of their ability to deal with complex adult reconstruction issues. The faculty does not need the fellow in order to function but do seek to train a highly motivated individual. The depth and breath of the year long program in the very “livable” city of Philadelphia maximizes education and quality of life on par with any similar program in the country.
