Informed Consent Forms

• Informed consent forms are required for genetic testing as well as parentage testing.



• Download one of our Informed Consent forms. Other consent forms are acceptable as well.

 Genetic Testing Informed Consent Form

• This policy is in accordance with the Final Report of The National Human Genome Research Institute’s Task Force on Genetic Testing (Promoting Safe and Effective Genetic Testing in the United States, Editors Neil A. Holtzman, M.D., MPH and Michael S. Watson, Ph.D, September 1997). The Task Force was created by the National Institutes of Health-Department of Energy Working Group on Ethical, Legal, and Social Implications of Human Genome Research.
