Application Forms & Instructions

ATTENTION: Change in SAIF application process.

Principle Investigators should give Sallie Livingston the following:

one complete signed original SAIF application
(+IACUC protocol & approval letter, etc)
an electronic copy of all of the required documentation
emailed to: sallie@mail.med.upenn.edu
(eg: SAIF application, IACUC approval letter, IACUC protocol, and, if appropriate IACUC amendment approval letter(s), IACUC amendment(s), and the AOS form).

SAIF Research Application:
This is the Application form that must be completed in order to gain access to the SAIF resources.

(AOS Form) Application Form for Holding of Animals in #509 Richards for Longitudinal Imaging Studies:
This is the Application From that must be completed in order to hold animals in the SAIF animal facilities for longitudinal imaging studies. (This must be filled out EACH time animals are brought into the facility)
