Application Process

Applications for the Craniofacial Plastic Surgery Fellowship beginning July 1, 2013, will be accepted beginning July 1, 2012 through September 15, 2012.

Interviews will be on September 22, 2012.

The fellowship runs from July 1, 2013 through June 30, 2014.

Completed Craniofacial Fellowship Application (PDF)
Curriculum vitae
Copy of USLME Step 1, 2 and 3 scores
Personal statement
Three letters of recommendation, including one from your plastic surgery program director
